DentAssure is a fully insured group dental plan for employers of 2 or more employees. Plans can be written as Voluntary or Contributory, but with Voluntary plans at least 25% of the company's eligible employees and dependents must enroll. Many options are available with the DentAssure Indemnity UCR Plan, the DentAssure PPO UCR Plan, and the DentAssure PPO Schedule Plan. The DentAssure Plan also offers a Vision product, which can be added as a rider to one of our dental plans or purchased separately.

DentAssure Indemnity UCR Plan
This is a group plan for 2 or more that offers the freedom to visit any licensed dentist of choice. Variable plan features allow you to select your deductible co-insurance limit and calendar year maximum benefit.(See Plan)
DentAssure PPO UCR Plan
This option offers the flexibility of choosing your calendar year maximum benefit and your calendar year deductible. In addition, you can choose to use a network dentist or a dentist of your own choosing. Both of our PPO plans use Dental Health Alliance, LLC as the PPO Network. Potential employers will be pleased with the coverage provided by DHA.(See Plan)
DentAssure PPO Schedule Plan
This is the ideal plan for the employer who has never before provided dental benefits for his or her employees. This plan is also economical, which is helpful to employers for whom cost is a major factor.(See Schedule)
DentAssure Vision Plan
Our Vision Plan provides employers with the opportunity to offer their employees the freedom to go to any vision provider they choose without receiving reduced benefits. (See Vision plan)
